Adults with Developmental Disabilities
The Adults with Developmental Disabilities waiver provides services and individualized supports to persons 18 years of age or older with a developmental disability who are at risk of placement in an Intermediate Care Facility for persons with Intellectual or Developmental Disabilities. Covered services include home accessibility modifications such as ramps, grab bars, and bathroom adaptations.
People who are Medically Fragile, Technology Dependent
The program provides Medicaid-funded supports that allow eligible children to live at home instead of in a hospital or skilled nursing facility. Services include private duty nursing, caregiver support, medical equipment, and home accessibility modifications such as ramps and bathroom adaptations.
Illinois’ Persons with HIV or AIDS waiver
The Persons with HIV or AIDS (HIV/AIDS) waiver provides services and individualized support to persons with Human Immune Deficiency (HIV) or Acquired Immune Deficiency Syndrome AIDS who live in the home. The Persons with Brain Injury (BI) waiver provides services and individualized support to persons with a brain injury who live in the home. Illinois’ Support Waiver for Children and Young Adults with Developmental Disabilities
The Support Waiver for Children and Young Adults with Developmental Disabilities provides services to children and young adults with intellectual or developmental disabilities ages 3 through 21 who live at home with their families and are at risk of placement in an Intermediate Care Facility for Individuals with Intellectual Disability (ICF/ID).
Fixing My Home
The program provides funding to units of local government and non-profit organizations throughout Illinois to provide home accessibility grants to residents. These grants for older adults and people with disabilities are designed to help people stay in their homes and prevent premature or unnecessary institutionalization. Funds are not provided directly to homeowners.
