The Alternatives Program (TAP)

The Alternatives Program of the Weber-Morgan Area Agency on Aging offers a long-term case management program arranging services for qualified individuals to avoid premature institutionalization in nursing homes or assisted living. Services may include home modifications. This program is for those age 60 or older however, 25% of clients can be age 18-59.
