Children’s Waiver Program
The Children’s Medicaid Waiver Program provides community-based services for children who are under age 18 who have a documented developmental disability and need medical or behavioral services at home. Supports include environmental accessibility adaptations.
Habilitation Supports Waiver
The Habilitation Supports Waiver (HSW) provides services that are enhancements or additions to Medicaid State Plan coverage for individuals with intellectual or developmental disabilities aged 0 or older who meet an institutional level of care. It can include environmental modifications for individuals.
MI Health Link Program
MI Health Link offers a broad range of medical and behavioral health services, pharmacy, home and community-based services, and nursing home care, all in a single program designed to meet individual needs. Services include but are not limited to: respite, adaptive medical equipment and supplies, assistive technology, chore, and environmental modifications.
MI Choice Waiver Program
The Michigan Choice Medicaid Waiver program provides services and support coordination in the home, including environmental accessibility adaptations. It is administered via Area Agencies on Aging and other home and community-based care agencies across the state of Michigan.
Single Family Housing Repair Loans & Grants in Michigan
Also known as the Section 504 Home Repair program, this provides loans to very-low-income homeowners to repair, improve or modernize their homes or grants to elderly very-low-income homeowners to remove health and safety hazards.
