Fall Prevention Program
The Fall Prevention Program of the Contra Costa Area Agency on Aging provides in-home Occupational Therapist assessments and evaluations for older adults at risk of falls and/or of being institutionalized due to inadequacies in the home. The program also provides assistive devices and home modifications that help older adults to remain safely in their homes.
Home Modification & Accessibility Grants
The Home Modification and Accessibility Grant is available to assist Alaskans with disabilities or who are Mental Health Trust beneficiaries. Housing Accessibility Improvement Program (HAIP) Grant Program
The HAIP Grant is funded through the State of Alaska Department of Health and Social Services in cooperation with the Alaska Mental Health Trust Authority. The grants fund accessibility improvements to existing homes to address functional limitations for residents with disabilities.
